Letter of Agency (LOA) Form

A Letter of Agency (LOA) is a legal document that authorizes Callifi, a telecommunications provider, to act on behalf of you or your business. This agreement permits Callifi to manage your telephone services and make necessary changes to your account, including the porting of phone number(s) to Callifi.

To ensure the successful completion of the LOA, it is crucial to accurately fill out the form with the correct information that matches the details listed in your current phone service provider's account. Additionally, a recent phone bill must be submitted along with the completed form.

If you have multiple accounts, it is necessary to fill out and submit a separate LOA for each account along with a corresponding phone bill.
